Vitamin D is a modern paradox. We live in the land of sunshine, yet Italian studies show that 50-70% of the population has insufficient vitamin D levels during winter. We inhabit a Mediterranean country but spend most of our time indoors, use sunscreen all day (rightfully, to prevent skin cancer, but it blocks vitamin D synthesis), and eat little fatty fish. The result is that one of the most important nutrients for overall health is systematically deficient in a population that should have it in abundance.

Vitamin D is about more than just bones

The discovery that vitamin D has receptors (VDR) on nearly every cell in the body revolutionized our understanding of this hormone (technically a steroid hormone, not a vitamin). Its documented functions go far beyond calcium metabolism: immune system regulation (both innate and adaptive), inflammation modulation, regulation of cell growth and differentiation, muscle function (deficiency causes myopathy with muscle weakness and increased fall risk in older adults), mood modulation (deficiency is linked to seasonal depression), blood pressure regulation, and pancreatic function (insulin synthesis and secretion).

How your body produces vitamin D

Skin synthesis is the primary source: UVB rays convert 7-dehydrocholesterol in the skin to pre-vitamin D3, which is then activated in the liver (25-OH D3) and kidneys (1,25-OH2 D3, the active form). Conditions that limit synthesis include: latitude above the 35th parallel (all of Italy is above this threshold) during winter—UVB rays aren't sufficiently angled from October to April to activate synthesis. Sunscreen use with SPF 15 or higher reduces synthesis by 99%. Dark skin: melanin acts as a natural sunscreen, reducing synthesis. Glass windows: glass blocks UVB rays. Clothing that covers most of the skin surface.

Food sources: limited but important

Vitamin D in food is relatively rare. Main sources include: wild salmon (600-800 IU per 100g), herring (1,628 IU per 100g, the most concentrated source), mackerel (360 IU per 100g), sardines (270 IU per 100g), egg yolk (40 IU per yolk), mushrooms exposed to UV light (UV light activates vitamin D2 production in mushrooms: "sun-dried" mushrooms can contain up to 400 IU per 100g), and milk and dairy products (often fortified). Only by eating fatty fish three times a week can you get a meaningful dietary contribution. It's impossible to meet your needs through food alone without fatty fish.

Optimal levels and how to measure them

Blood 25-OH vitamin D (25-hydroxyvitamin D) is the most commonly used status marker. Levels below 12 ng/ml (30 nmol/l): severe deficiency. 12-20 ng/ml: insufficiency. 20-30 ng/ml: borderline levels. 30-50 ng/ml: adequate levels. 40-60 ng/ml: optimal levels for most documented benefits. Above 100 ng/ml: risk of toxicity (hypercalcemia). The test costs just a few euros and is reimbursable in some regions for at-risk groups. It's the only way to know concretely whether you need to supplement.

Supplementation: doses and forms

Vitamin D3 (cholecalciferol) is the recommended form because it's more effective at maintaining serum levels compared to D2. Guideline doses for documented deficiency: 2,000-4,000 IU daily for healthy adults with borderline levels, 4,000-6,000 IU for moderate deficiency (always under medical supervision). Don't self-prescribe doses above 4,000 IU daily without a blood test: vitamin D is fat-soluble and accumulates. It's absorbed better with the meal richest in fat during the day.

The sun protocol to maximize natural synthesis

From April to September in Italy, sufficient sun exposure means 15-30 minutes of direct sunlight on arms and legs (not through glass or with high SPF protection) between 10 a.m. and 3 p.m. This is enough to synthesize 1,000-3,000 IU daily in fair skin. Never burn: UV damage isn't justified by vitamin D synthesis you can safely obtain through supplementation. After 15-20 minutes, apply sunscreen for prolonged exposure.

How to tell if you're probably deficient without testing

High probability of deficiency if: you never go out without sunscreen even in summer, you have very dark skin, you never eat fatty fish, you're older (skin synthesis decreases with age), you're significantly overweight (vitamin D accumulates in adipose tissue, reducing bioavailability), or you have conditions that reduce intestinal absorption (celiac disease, Crohn's disease). In these cases, it's worth getting tested rather than waiting for symptoms.

Fall supplementation: when to start and how

In Italy, the window for effective sun synthesis closes around October-November. From that point, without supplementation, levels progressively decline until their lowest point in February-March. Start supplementation in October: 2,000 IU daily of vitamin D3 with your evening meal rich in fat. Continue until April-May (when the sun becomes effective again). If possible, get a blood test in November and another in March: comparing the two tells you if your dose is adequate.
